• You MUST re-enroll for Flexible Spending accounts if you plan to keep this benefit in 2009. No paper forms are being accepted this year. You MUST enroll online.

  PLEASE NOTE:

  • The College is changing dental vendors this year. The new vendor will be Cigna.

  • DeltaCare USA (DHMO) participants MUST complete a form to select a new dental office from the Cigna DentalCare network.
  
  
  
  • DeltaCare USA is the HMO dental plan that requires that you preselect a dental office from the list. If you are in this plan you will receive a letter at home with instructions. PPO dental participants can see any dentist anywhere. They do not need to complete enrollment forms, and will automatically be enrolled with Cigna. If you are unsure which plan you have, please contact our office.
    
  • Other than those participants in the HMO dental plan, if you are not making any changes to your benefits, you DO NOT need to turn in an enrollment form. Your elections will stay the same as in 2008.
