Weight Watchers at Work

Congratulations on making a commitment to your health and wellbeing. You are to be commended. As you know you may utilize EAP funds to be reimbursed for participation in Weight Watchers at Work.

In order to be reimbursed for the program you must attend a minimum of 10 sessions on campus.

Your attendance each week must be recorded in your “Membership Book” which must be stamped by the Weight Watcher’s Leader when you attend the meeting on campus. Weight Watchers will accept payment by cash, check or credit card for the program.

Every employee must pay Weight Watchers directly. If you meet the attendance criteria you may submit a request to be reimbursed under EAP. Some employees are concerned about disclosing their weight which is recorded beside the date of attendance on the “membership book”. You may block out the weight information on the book when you request reimbursement. All we need from you are dates of attendance in order to be reimbursed under EAP.

In order to be reimbursed under EAP you must provide Lori Stegeman , Administrative Aide in the office of Human Resources with three items.

  1. An EAP form signed by your supervisor.
  2. Proof of attendance at 10 (weekly) sessions during the duration of the program.
  3. Proof of payment in the form of a “canceled check (front and back) and a copy of the attached “Weight Watchers At Work Member Log” with Payment Type circled and signed by your Weight Watchers Leader
  4. OR

  5. Credit card statement indicating payment made to Weight Watchers and a copy of the attached “Weight Watchers At Work Member Log” with Payment Type circled and signed by your Weight Watchers Leader
  6. OR

  7. If paying cash, a copy of the attached “Weight Watchers At Work Member Log” with Payment Type circled and signed by your Weight Watchers Leader.
  8. Please note: The only signatures that will be accepted by the Finance Office as proof of payment are those of the WW Leader.

Lastly we encourage you to attend the meetings at work because WW has found that people who attend WW meetings lose three times more weight than those who go it alone.
